Is there a difference between a squad of 10 CSM's armed with an icon of Nurgle and a squad of 10 Plague Marines?
I don't see the difference, but why are there special Plague Marine models and then chaos space marine models?
Another question would be, do you have to convert the chaos space marines in any way to make them just plague marines? (no icon required?)

Plague marines have blight grenades and feel no pain, I believe. Yes you would have to convert normal CSMs in order to be plague marines (which would be easy, just greenstuff some pus and boils on them).

Yeah there is often a few extra's to the cult troops, than compared to Chaos Space Marines with an icon. Like Durandal says above - Plague Marines have FnP and Blight Grenades.

As well as being Fearless :) some would call that good, some bad.
All cult troops having a couple extra tidbits that you can't get as regular CSM.

Basically, they are totally different. A quick read of your codex would have told you that.

yes yes, i see this now, thank you. Basically plague marines are tougher, but a little slower in CC. That's where I'll incorporate some berserkers to make up some CC power.
